Standard Position

Patient Care Solutionist / Patient Care Assistant

Position Purpose

Provides compassionate, non-medical support services that promise independence, dignity, safety, wellness, and quality of life for individuals receiving services through Unified Freedom Solutions LLC.

Post-Hospital Discharge Support

Provides non-medical hospital-to-home transition support, including safe settling into the home, follow-up appointment assistance, meal preparation, medication reminders, grocery/household support, family communication, and reporting changes or concerns to the supervisors.

UFS Pay Range (Hourly)
$18.00 - $21.50
Premium Pay Opportunities

Essential Responsibilities

Personal care assistance

Patient Care Solutionist Specialist – Dialysis Support

Position Purpose

Provides specialized non-medical companionship or support to individuals receiving dialysis services before, during, and after treatment, including post-hospital discharge support when included in the client service plan.

Post-Hospital Discharge Support

Provides non-medical hospital-to-home transition support, including safe settling into the home, follow-up appointment assistance, meal preparation, medication reminders, grocery/household support, family communication, and reporting changes or concerns to the supervisors.

UFS Pay Range (Hourly)
$20.00 - $23.50
Premium Pay Opportunities

Dialysis Support Responsibilities

Pre-dialysis preparation and verification
Transportation coordination and escort support
During dialysis companionship and advocacy
Emotional support and patient engagement
Post-treatment monitoring and recovery support
Post-routine memory support
Rush-reminder support

Documentation and reporting
Communication with care team members

Unified Freedom Solutions LLC provides personalized, non-medical home care services for seniors, individuals with chronic conditions, those recovering from hospitalization, family caregivers needing respite, and dialysis patients through our Freedom for Families™ Dialysis Companionship Program.

During each visit, a Unified Freedom Solutions LLC Patient Care Solutionist provides compassionate, non-medical support tailored to your needs. Services may include personal care, companionship, meal preparation, medication reminders, housekeeping, transportation, and safety monitoring.

Our Freedom for Families™ Dialysis Companionship Program also offers supportive companionship before, during, and after dialysis treatments.

4. Is home covered by insurance?

Coverage for home care services depends on your insurance, eligibility, and the type of care needed. Unified Freedom Solutions LLC can help you explore available payment options, which may include Medicaid Waiver Programs, VA benefits, long-term care insurance, private pay, and other approved funding sources.

We offer complimentary consultations to review your situation, discuss eligibility, and create a personalized care plan that fits your needs.

6. Are you providers certified and experienced?

Yes. Our caregivers are carefully screened, trained, and experienced in providing compassionate non-medical home care. Team members may include CNAs, CPR/First Aid-certified caregivers, and professionals trained in client safety, elder care, dementia awareness, and dialysis companionship support.

8. Can I choose my Care provider?

Yes. Clients are encouraged to continue seeing their existing physicians, specialists, therapists, and dialysis care teams. As a non-medical home care provider, Unified Freedom Solutions works alongside your healthcare professionals to support your comfort, independence, and overall well-being.

9. What if I need urgent or emergency care at home?

Unified Freedom Solutions does not provide emergency medical care. In a medical emergency, caregivers will follow agency procedures, including contacting 911 and notifying designated family members or emergency contacts. We also provide post-hospital support services to help clients recover safely and comfortably at home after hospitalization or emergency treatment.
